Application Process

Proposal must be structured and include the following:

1. Grant Application.
   Download Acrobat Fillable Form
   Download & Complete by Hand
2. History and goals of the organization.
3. Detailed description of the project.
4. Target and action plan of project, and expected results. Be sure to include project budget.
5. List of other organizations contacted for support of this specific project proposal, and the amount(s) requested from them.
6. Percentage of revenues coming from federal, state, and local governments.
7. List of current Board of Directors.
8. Tax exempt letter from the Internal Revenue Service.
9. The most recent audited financial statements and IRS form 990.
10. Any additional information that would support the application.
